HC Terps 2025 Medical Form Signed by a licensed physician

HC Terps Code of Conduct Signed by parent(s) and athlete

State ID Card (Must be obtained from the MVA (Maryland Department of Motor Vehicles. Appointment required. Current out of state ID cards accepted.)
